Health Certificate (Type B) and NCKU Freshman Medical Check-up

All NCKU Freshmen are required to have a medical check-up for the university enrollment. The students who fail to do the medical check-up here will face the deprival of student status.

For the students who have a Visitor VISA (with annotation “FS and the name of school”) when coming to Taiwan, they have to apply for a Resident VISA for students very soon after enrollment at the university, or before their Visitor VISA runs out. To apply for a Resident Visa in Taiwan, it is necessary to have a Health Certificate (Type B) issued by one of the designated local hospitals in Taiwan. If you are the student who has a Visitor VISA, please remember to bring the proof of positive measles and rubella antibody titers, or measles and rubella vaccination certificates to Taiwan (if available).

★NCKU Freshman Medical Check-up

Ø          For the international students those who have Resident VISA

Ø          Location: NCKU Hospital (The timetable would be announced on your enrollment day.)

Ø          Special Price: NTD$800

(It is the special price in the appointed duration for the NCKU freshmen, if you don’t join it, the price of the medical check-up outside will be much more expensive than that provided by the school.)

★Health Certificate (Type B)

Ø          For the international students those who have Visitor VISA

Ø          Location: NCKU Hospital (The timetable would be announced on your enrollment day.)

Ø          Special Price: NTD$1,338 (The special price is in the appointed duration for the NCKU freshmen.)

(Including both Health Certificate and NCKU Freshman Medical Check-up)

Ø          If you don’t have any measles and rubella vaccination certificates with you in Taiwan, you have to take the vaccine in NCKU hospital or private clinics. After you return the record to NCKU hospital, you could complete Type B Health Certificate.

Ø          Information to take three-in-one vaccine (Measles, Rubella and Parotitis)

1.        Bring the passport and pay the fee by yourself

2.        Go to NCKU Hospital: Fee - around NTD 450

3.        Go to Syu Cing Jhong Clinic 徐慶鐘小兒專科診所 : Fee - around NTD 150 (from 15 years old to 49 years old Female ONLY)
